At Home and Care, we offer clinical nursing and personal care services to NDIS participants, with the aim of maintaining their health and independence at home. Our nursing services are provided by a highly experienced registered nurse, who leads a team of caregivers that deliver personalized and high-quality care in the comfort of your own home.

Our range of Services includes:

• Visiting nurses who provide healthcare services and support to patients in their homes.
• Medication set-up and administration/supervision to ensure that patients take their medications as prescribed.
• Catheter care, including regular cleaning and monitoring for infections.
• Continence advise, including assessments and strategies to manage urinary or fecal incontinence.
• Wound care, including assessment, treatment, and management of various types of wounds.
• Stoma care, including maintenance and management of surgical openings.
• Monitoring of vital signs, such as blood pressure, pulse, temperature, and blood sugar levels, to identify any changes in a patient’s health status.
